Practice Guide Development: Strength-Based "What Matters" Conversations - Learning Disability

Development of a simple guide for practitioners aimed at supporting meaningful, strength-based "What Matters" conversations within the context of social care in Wales. The guide will be specifically targeted at practitioners or key co-ordinators involved in care and support planning. This may include named key workers, care co-ordinators, senior care workers or managers and social workers.
