Intermediate Care - Complex Older People's Mental Health known as OPMH+

OPMH+ in Somerset is the Discharge service to support recovery, reablement and assessment and is the route for supported discharges where a person is unable to return to their usual place of residence at the point of discharge from acute hospital. The Bedded pathway model ensures no assessment for ongoing care happens in an acute hospital setting but happens in an appropriate pathway bed.
