Reablement Care Service

The Reablement Care Service shall provide short-term, outcome-focused support to eligible Enfield residents who require assistance to regain, maintain or improve their independence following discharge from hospital. The externally commissioned support will focus on hospital discharge pathways only and will be underpinned by a strong reablement approach.
