The Provision of Self-Contained Apartments for Enablement Services with Extra Care Scheme

This agreement relates to the utilisation of 6 self-contained apartments for short term enablement services from hospital. The apartments are to be made available by the housing supplier to support with hospital discharge processes, where there are both care requirements and accommodation requirements. There will be exclusive use of these apartments for the duration of this contract at the Stirchley House Extra Care Scheme. The apartments will be provided unfurnished.
