Direct award to Thames Reach to deliver a service targeting those at risk of long-term rough sleeping. The Prevention Pilot aims to equip day centres, faith groups, and other frequent referrers with tools and guidance to prevent rough sleeping, while a dedicated team will casework individuals who self-identify as at risk.
