DA0026- High Intensity Use Pilot

Pilot model built on the success of the homeless engagement nurse that is currently funded via the DATRIG Housing Support Grant. This funding will enable that nurse to be increased to a band 7 in order to supervise the recruitment of 1 care coordinator, and 2 peer support workers with additional GP resource.
