Hospital Discharge and Peer Mentoring Service

The Hospital Discharge and Peer Mentoring service will work closely with key partners to reduce the likelihood of individuals being re-admitted into hospital within 30 days and offer support, both professionally and peer-led, to individuals who have been admitted to hospital with alcohol misuse related problems and to ensure that individuals with a treatment need enter structured pharmacological or talking therapy treatment.
