Complex Care FPS for Individuals Resident in, or funded by St Helens

The support is based on a recovery and rehabilitation focused model, with all service users receiving a core package of support from a staff group. Support received will be delivered as stipulated in each service users Social Care Assessment and individual Care Plan.
