The Home from Hospital service is a highly collaborative and coordination-focused service providing, flexible practical support (not personal care) for a period of up to 4 weeks. The service is for vulnerable Wiltshire residents (aged 18+) who are medically fit for discharge but unable to return home due to practical barriers.
