Homeless Health Programme (Kensington & Chelsea and Westminster)

1) This model delivers a clinical outreach programme which takes healthcare services to individuals with a view to improving access to primary care for those on the streets. With access to 4 commissioned hostel beds, the GP practices deliver a clinical programme which is responsive to changing need and circumstances.
