High Intensity Need - Enhanced Social Prescriber Service

It was agreed to utilise health inequalities funding to add capacity to existing services within Barnsley – with a view to strengthening the support on offer to patients with the greatest need, optimising pathways between these services and enabling the most vulnerable and complex residents to receive care within communities rather than within the hospital / ED setting.
