Adults Outreach Service

The Adults' Outreach Service is commissioned to enable people with assessed needs to lead a full life, maximising independence, promoting health and wellbeing and supporting people to remain safe and connected to local communities. The service is designed to support needs, and those with more complex needs, in an innovative way.
