Integrated Community Care System Consultancy (ICCSC)

Cardiff and Vale University Health Board is seeking to engage a management consultancy to support the transformation of its primary and community care model. The appointed consultancy will provide strategic guidance, implementation planning, financial modelling, benefits identification and realisation and stakeholder engagement to accelerate the transition to a population health-focused, integrated community care system.
