AACC E2E Digitisation

This is an amendment to the previous UK6 notice (Notice identifier: 2025/S 000-053853) This procurement will identify a Bidder who will provide a digitalised All Age Continuing Care (AACC) end-to-end digital system, where information seamlessly flows between processes by connecting the entire lifecycle into one platform.
