The Local Government Association delivers the BCF Support Programme, providing support to local areas with the integration health and care services. The Programme is seeking a contractor to provide targeted support to this System to improve both outcomes for people and for resource use in systems by reducing emergency admissions, discharge delays and care home admissions.
