GP/Dental Integrated Diabetes Pathway Pilot

The provider is the recognised provider alliance of general practice in NCL, providing primary health care services. Patients who meet the criteria for treatment under this contract (and who will subsequently be referred for additional treatment) will be identified from patient lists held by local general practices in North Central London. The provider is the only provider capable of delivering the services due to the nature of those health care services.
