Reablement Services

Cheshire West and Chester Council are currently reviewing their reablement process for individuals transitioning from hospital to home-based domiciliary care. To assist with our soft market testing, we would appreciate your feedback via the questionnaire published on the Chest, www.the-chest.org.uk.
