Co-Produced Dementia Support Programmes

The planned procurement is to recommission a service which facilitates a range of support for people living with dementia in the community, with a focus on the person living with dementia themselves and connects those that have an unpaid caring role for them into appropriate support offered by or in partnership with other services.
