Frailty Care Navigators - Oxleas

The Trust has a requirement for care navigators to work within the Greenwich Frailty team. The role is social prescribing for Frail older people in care homes throughout the borough of Greenwich. This role is within the purview of voluntary sector organisations normally with a specialism in older peoples’ services.
