NHS Health Checks Service (Direct Award Process A)

The Borough Council of Calderdale wishes to commission the mandated NHS Health Checks service for Calderdale to be delivered by a single or lead provider. The service will be based around delivery in general practice plus some community delivery outside of general practice with resources targeted to people who are likely to benefit the most.
