NHS Lincolnshire Integrated Care Board are inviting suitably qualified and experienced providers to express their interest in delivering a Diabetes Structured Education Programme.<br/>The aim of the service is to facilitate and provide the people of Lincolnshire with a practitioner-led education programme to support the management of Type 2 diabetics. The service should offer both face-to-face and online/video support to patients across the County including areas of high prevalence and deprivation.
