The UCP is predominantly a software service that enables the sharing with urgent care services (111, London Ambulance Service, Emergency Departments in Acute Trusts) of treatment escalation plans, crisis care plans, end of life care plans, patient specific protocols and personalised care plans that have been co-developed and documented by different health and care disciplines.
