Rapid Response Care at Home

The Rapid Response Service will facilitate the safe and effective discharge of service users from hospital who have been declared as medically fit for discharge but who may still have care needs that can be met in the service users own home. Additionally, the service will support people at risk of unplanned hospital admission, carer breakdown and crisis/ high risk cases.
