North Staffordshire & Stoke on Trent Memory Support Service

North Staffordshire CCG and Stoke on Trent CCG are wishing to procure memory support service. This service will provide support to patients and carers who are awaiting or have received a dementia diagnosis through the provision of a named care co-ordinator following the dementia well pathway.
