NHS NW London ICB requires the provision of Hospital to Home Services in Kensington and Chelsea, Westminster and Hammersmith & Fulham starting 1 April 2023. The aim of the service is to help people safely discharged home where they do not have a carer/ someone (e.g. family) who can look after them & they do not fall under statutory services remit but require support.
