The Provider will deliver a health, wellbeing, lifestyle and behaviour questionnaire for Dudley's children and young people on behalf of Dudley Council, to be carried out biennially in mainstream primary and secondary education settings, colleges, and in settings for children with additional needs by pupils in year 5, 6, 8, 10, 12 and 13.
