Nottingham City CCG Enhanced Physform Project Outcome Measures Pilot

The Enhanced Physform pilot aims to improve the physical health of patients with serious mental illness (SMI) and address known health inequalities. Patients with an SMI are reported to have a reduced life expectancy of up to 25 years compared to the general population, an inequality that is mainly related to patients with cardiovascular, respiratory diseases, diabetes and increased risk of cancer and liver disease.
