Provision of Discharge to Assess Beds

Tameside is introducing a Discharge to Assess (D2A) scheme to support the hospital discharge process helping to alleviate pressure within the acute sector by facilitating the discharge process through the provision of temporary support in a residential/nursing home setting for service users medically fit for discharge.
