NHS Herefordshire Clinical Commissioning Group Individual Care Package

The service will deliver a 24-hour nursing care package for a service user with complex health needs. The service user has an acquired brain injury and requires support at home where she lives with her family in adapted accommodation. The individual care service is required for 7 days per week, 365 days per year. Additional information: The contract contains one 2 year option to extend. The stated contract value excludes any extension period.
