NHS North West London CCG is commissioning for the provision of Specialist Palliative Care to patients with progressive, incurable disease for whom death is foreseeable. The majority of patients will have end stage conditions from cancer and HIV, as well as to patients with Motor Neurone Disease, Heart Failure and COPD.
