Waltham Forest CCG Anticoagulation Services Community Pilot

One prime provider will manage all aspects of community anticoagulant provision for Point of Care testing (POCT) and monitoring of stable patient INR across Waltham Forest via the use of hubs, which include GP Practices/ Community Clinics and Pharmacies.
