Kent Community NHS Health Foundation Trust (KCHFT) is seeking tenders from suitable experienced providers who can deliver a Home with Support Service. The Home with Support Service supports the discharge of patients who have been assessed in the Acute as safe to go home by receiving a further assessment of need, rehabilitation and a period of recovery at home and develop a plan of care and onward referral if required.
