18 168 - Rapid Response Care at Home to support and facilitate hospital discharge or prevent admissions

Cheshire East Council wishes to appoint a provider for a Rapid Response Care at Home Service to alleviate winter pressures, supporting the discharge of patients who are medically fit from hospital to home, but who require additional support to enable them to make this transition effectively and safely.
