PROVISION OF REHABILITATION, REABLEMENT AND TRANSITIONAL CARE IN A CARE HOME

Hartlepool Borough Council, in collaboration with the Stockton & Hartlepool CCG, seeks to commission a rehabilitation, renablement and transitional care service through the block purchase of 20 beds in a designated unit within a registered care home in Hartlepool.
