Stroke Early Supported Discharge Service

NHS Ipswich and East Suffolk and West Suffolk Clinical Commissioning Groups are looking to commission a Stroke Early Supported Discharge (ESD) Service across the Ipswich, East Suffolk (minus Waveney) and West Suffolk localities.
