Establish underlying causes of accident including (but not limited to) • suitability of tree assessment before carrying out cutting operation • was the work properly planned • method of work used for felling the tree • training of employees undertaking the tree work • supervision of tree felling activity • should this incident have been preventable • How far below the minimum legal standard the company fell. To keep HSE updated on progress with email correspondence with Inspector every 2 weeks.
