Clinical engagement to increase use of Coordinate My Care in London

NHS South West London Clinical Commissioning Group (the CCG) is acting for itself and for CCGs across London to run a project to increase the use and documentation of urgent care plans on Coordinate My Care (CMC). £200k funding is budgeted to secure time limited clinical and engagement leadership.
