Evaluation partner for CYP Diabetes transition pilots

The appointment of Ipsos MORI to work as an evaluation partner to work with up to 21 pilot sites between September 2022-March 2025 to identify key scalable themes and learning to underpin improvements in diabetes transition and young adult care.
