Adolescent Healthyweight and lifestyle service

To develop, deliver and evaluate a service to support healthy weight management and healthy living for adolescents aged 14 years to 19 Years ( up to 25 years SEND). The service will form an integral part of the childhood healthy weight management action plan providing a tier 2 weight management service for those needing addtional support.
