Phase 5 of the pan-London CHC in nursing homes AQP contract

Phase 5 of the pan-London CHC in nursing homes AQP will open for applications in June 2016. Successful providers will be added to an AQP list which will be used by London Clinical Commissioning Groups (CCGs) and discharge teams to make continuing healthcare placements in nursing homes.
