Mental Health Support Services Framework Agreement - Call-off 6 Moving Forward On-line and Home Fund

The service will be provided using NHS funding for post-discharge support for mental health patients in Nottinghamshire and Nottingham City. This funding enables the Provider to identify any blockages to discharge where a person is 'clinically well enough' leave inpatient care and support people to overcome those difficulties and facilitate a timely return to the community.
