Provision of Funded Nursing Care Assessments to Banes, Swindon and Wiltshire CCG

Bath and North East Somerset, Swindon and Wiltshire Clinical Commissioning Group are seeking to commission a provider to carry out Funded Nursing Care ('FNC') Assessments for c670 individuals.
