Bromley CCG is seeking to appoint an organisation for the provision of a service delivering community cardiac rehabilitation and exercise referral programmes. The community Cardiac Rehabilitation Programme is to support a hospital based phase 3 and deliver phase 4 of an accredited BACPR approved cardiac rehabilitation programme across Bromley.
