NWL CCG requires the Provision of Community Independence Service (CIS) in Hammersmith & Fulham, Kensington & Chelsea and Westminster. The aim of the service is to provide service users access to: -A community based rapid response service (within 2 hours, up to 5 days length of stay in the service) to prevent hospital admissions -A Community rehabilitation and reablement services (for up to 6 weeks of input).
