KMCIT-105 - Healthy Ageing Tool

Provision of a digital tool to support self-care and help people to maintain their independence. A digital tool that helps people understand clearly how they are ageing, provides them with appropriate advice, offers them access to activity resources and enables them to monitor their progress.
