Oxfordshire Out of Hospital Care Model - Step Down Service

This Contract is for the provision of a Step-Down service in Oxfordshire. The provider is to deliver both the accommodation and staff. Step-Down housing is classified as Discharge to Assess (D2A) Pathway 2 intermediate care. It is short-term accommodation (typically up to six weeks) for people who are ready to be discharged from hospital but who are either homeless, threatened with homelessness or whose housing is not suitable to meet their needs (for example it may require major adaptations or a deep clean).
