Alzheimer's Society will deliver this project across South Gloucestershire and include the following services: Dementia Support Service (30 hours) - working with Sirona, to provide and deliver a countywide service. The Alzheimer's Society lead on Clusters 4-6, whilst Sirona concentrate on Clusters 1-3. Working with Sirona, to provide information and sign-posting cover across the area, when needed.
