Hub for People with Young Onset Dementia (Under 65s) and their family carers

The service will cover all areas of Sheffield for the period 1st July 2020 - 31st March 2025 (4 years and 9 months). We are looking for a creative and innovative Provider who will develop a high quality Young Onset Dementia 'Hub' to meet the needs of people with young onset dementia and their family carers. The 'Hub' is not expected to be a physical building but is expected to serve people from across Sheffield so would offer activities utilising different existing locations across the city.
