Ukraine response: new patient health questionnaire

Building on current new patient questionnaires available in general practice and other relevant materials , develop a new patient questionnaire which can be issued to and used by GP practices to support the initial health assessment and management of newly registered vulnerable migrants including people seeking asylum and those arriving under Government relocation schemes.
