Homeless Care Navigation & Hospital In-Reach Service (Central London)

The NHS Central London Clinical Commissioning Group (the CCG) is looking for non-clinical in-reach workers, based in each of the London hospitals that serve the CCG population, to identify homeless patients at the point of admission.
